Colonoscopy has been touted as something everyone should do, and widespread screening is the best way to reduce colon cancer death risk. Everyone should get one. | The largest/best study to date shows no reduction in deaths due to colonoscopy versus alternatives. Much of the world is reluctant to waste time/money on this invasive procedure, when cheaper and less invasive stool sampling is nearly as good (and more likely to be done). |
I'm not sure I'm ready to completely attack the norms based on one study. But it makes sense.
- Visual is not significantly better than looking for chemical markers of stool samples
- The less invasive procedure is more likely to be done, more frequently. (And that offsets qualitative losses)
- There's less risk of complications (perforations, infections, etc) with the less invasive procedure
- Pateint discomfort is physchological "harm". We should minimize that for all but the highest risk people.
- There are CERTAINLY some high risk cases, or people with polyps, where this better diagnostic might help. So I'm not saying the procedure is bad. Just that as a widespread screening, it's a great way to make money, without actually improving outcomes.
So while I found the procedure not that horrible an experience (the prep being the worst part) -- I do think things that can reduce human suffering and wasted money/diagnostics, is a good thing. Of course, I'll also wait until there's more confirmation for this study. But like bloodletting and many other "good ideas", the sales pitch might have been dramatically overstated the outcomes.
